VACCINATION GUIDELINES: To ensure the protection of all pets under our care, the following vaccinations must be up to date. By admitting your pet for boarding, you give permission to update any vaccines deemed necessary, inclusive of a necessary physical exam. DOGS: DHLPP (Distemper and Parvo), Bordetella (Kennel Cough) Rabies and Flu. CATS: FVRCP (Distemper), Rabies, FELV (Feline leukemia - if outdoors)
I have read the above boarding agreement and understand and agree to the terms of boarding my pet with WBVC. I fully intend to pick up my pet on the date specified. If circumstances regarding my pick-up date should change, I wll notify WBVC as soon as possible.
